Join the Duett Community and connect with Care Managers and Clients who need your services.

Duett exists to create and support enduring relationships. To do this, we created a technology platform that more seamlessly enables Hoosier patients to connect through care managers to home health services.

Home health providers are invited to subscribe and begin taking cases from the Duett platform. To begin the onboarding process fill in the webform in step 1 below.

NOTE: The process below starts with the Webform that you need to fill out. The rest is automated. Once you fill in the webform, legal documents will be sent directly to your email. Payment for your subscription is included in the same process as signing the legal docs.

1) Fill out our Provider Webform

This is the first step to joining Duett and using the platform to build relationships with care referral organizations and to coordinate new clients.

2) Sign our Terms & Conditions and Business Associate Agreement

NOTE: This will be emailed directly to you for signature by DocuSign, once you have completed step 1.

3) Pay Duett’s subscription fee

NOTE: Paying for the Duett subscription is included in the same process as Step 2 and signing Terms & Conditions and the Business Associate Agreement.

There is an annual subscription fee of $600.00.

This can be paid:

Monthly - $50.00 per month
Quarterly - $150.00 every 3 months
Bi-Annually - $300.00 every 6 months
Annually - $600.00 every year

4) Duett will create your Provider account

We will handle this part! We will use the information from the webform to make sure your account is set up with the right geography, funding sources and service types your organization offers.

5) Access Duett and Engage with Care Referral Organizations

Once, we have created your account, you will receive a message from Duett, which will allow you to access the platform. From there, we invite you to engage with Care Managers and offer your organization as a provider to clients looking for services.
